LL-37 is a naturally occurring, 37-amino-acid antimicrobial peptide that serves as a vital component of the human innate immune system. Derived from the cathelicidin protein, it acts as a first-line defense against pathogens while regulating inflammation, promoting angiogenesis, and accelerating wound healing.

Core Functions

  • Antimicrobial Activity: It demonstrates broad-spectrum activity against bacteria, fungi, and viruses. It destroys pathogens by penetrating and disrupting their cellular membranes, making it a subject of research against antibiotic-resistant strains.

  • Immunomodulation: LL-37 helps modulate the immune system, often balancing inflammatory responses. It can act as both an anti-inflammatory agent and a pro-inflammatory mediator depending on the tissue microenvironment. [1, 2, 3]

  • Wound Healing & Tissue Repair: It promotes the migration of epithelial cells, induces the formation of new blood vessels (angiogenesis), and attracts necessary immune cells to sites of injury.

Research and Medical Applications

While naturally produced by epithelial cells and white blood cells, the synthetic form of LL-37 is being studied for various therapeutic applications:

  • Chronic Wounds: Research has shown it can aid in the closure of stubborn wounds and ulcers.

  • Infections: It is heavily researched as an alternative or complement to traditional antibiotics, specifically in breaking down bacterial biofilms.

  • Cancer and Neurobiology: Studies indicate LL-37 plays a complex, dual role in cancer (exhibiting both anti- and pro-cancerous properties) and has been linked in research to microglial activation and neuroinflammation in Alzheimer's disease models.

Safety and Regulatory Status

Despite its promising in-vitro properties, synthetic LL-37 has not received FDA approval for general medical use as an antibiotic. High concentrations of the peptide can be cytotoxic to healthy human cells, and its systemic levels can fluctuate during viral infections and systemic inflammation.
